Inpatients at St. Joseph Medical Center may be visited by Integrated Care Management (ICM) personnel to determine home care needs and eligibility. ICM personnel may assist in arranging home care needs through Upper Chesapeake/St. Joseph Home Care.
Services include:
Home Health: Provides experienced staff to administer healthcare services in the home setting. Specialists include: registered nurses, home health aides, physical therapists, occupational therapists, speech therapists, and social workers. To be eligible for home health services, patients need to be homebound, require a skilled professional service, be under the care of a physician who will order home care and be kept informed of the patien's condition, and live within the licensed territory.
Choice Care: Gives patients the opportunity to live independently at home while receiving the daily care or companion services they need. Choice Care can provide help with daily living activities, such as bathing, dressing and walking. Services provided include: certified nursing assistants, companions, educators, and Lifeline (a special service offering daily communication support for independent living at home).
Harford Hospice: Provides support for persons in the final stages of a life-limiting illness to ensure they live as fully and comfortably as possible. Hospice care is a coordinated program of palliative care and support services, including physical, psychosocial and spiritual care. Bereavement services are available for survivors, including children. Care is provided by a medically-directed interdisciplinary team that includes: registered nurses, social workers, volunteers, home health aides, dietitians, physical therapists, occupational therapists, and speech therapists.
To be eligible for Harford Hospice services, patients need to be in an advanced stage of a life-limiting illness, be under the care of a physician who understands and will participate in the hospice concept of care, have a competent person willing to assume responsibility for them in their home if necessary, be aware of the diagnosis and prognosis, and live within the licensed service area.
